I/we understand that this change relates to the licensee responsible for providing financial services and advice

I/we do NOT wish to continue our financial planning relationship with Chalmers Private Wealth under AFPG

By selecting the above, I/we confirm that:

I/we understand that this decision means we will no longer be a client of Chalmers Private Wealth

I/we understand that our adviser will no longer provide financial advice or ongoing services to us from 1 July 2026

I/we understand that any existing service agreements, including ongoing fee arrangements, will cease from this date

I/we understand that Chalmers Private Wealth and/or AFPG will notify relevant product providers to remove our adviser from our accounts

I/we understand that we will need to make alternative arrangements if we wish to continue receiving financial advice

I/we understand that this instruction applies to all financial planning relationships held with Chalmers Private Wealth, including any associated entities (such as companies, trusts, and superannuation funds) for which we are authorised to provide this instruction
